Referral Pathway

Eligibility

  • Adjudicated or high risk for commitment; serious/chronic behavior
  • Caregiver participation feasible; clinician safety criteria met
  • Screened by risk/needs tool; mental health triage for co-occurring needs

Process Notes

  • 24/7 therapist availability; low caseload; fidelity monitoring
  • Strong coordination with probation and school re-engagement
  • Step-down planning and short aftercare check-ins

Outcome Metrics (Stub)

  • Initiation, completion, and caregiver engagement
  • Complaint/violation recurrence at 90/180/365 days
  • Attendance/credit recovery; MH/SUD service completion when referred
