Logic Model
- Population: youth returning from placement (parole/aftercare).
- Intervention: dedicated coach; school enrollment + records; MH/SUD follow-up; family support.
- Mechanisms: accountability + practical help + faster problem resolution.
- Outcomes: school persistence; service completion; fewer violations/complaints.
Implementation Notes
- Begin pre-release; first in-person within 72 hours of return.
- Caseloads ≤ 15; 6–9 months typical; planned step-down.
- Handoffs documented (education, MH/SUD, ID/docs, transportation).
Outcomes (Schema)
- Initiation, Completion, Improvement (attendance/credits, service completion).
- Recurrence at 90/180/365 days; school persistence status.
