  • Caseloads ≤ 15; 6–9 months typical; planned step-down.
  • Handoffs documented (education, MH/SUD, ID/docs, transportation).

  • Initiation, Completion, Improvement (attendance/credits, service completion).
  • Recurrence at 90/180/365 days; school persistence status.
